디지털치료기기 RWE활용..."DB 변경 간소화해야"

국산 디지털 치료기기 업체들이 임상시험 단계에 진입하고 있는 가운데, 실사용 데이터(Real World Data, RWD) 활용에 대한 현행 제도 개선이 필요하다는 의견이 제기되고있다.
품목허가 획득 후 데이터베이스(Database, DB) 변경은 식품의약품안전처 의료기기 제조허가변경 심사·승인 대상인데(60일), 이 기간이 RWD 및 실사용 근거(Real World Evidence, RWD)를 통한 신속한 대응을 어렵게 할 것이라는 우려에서다.
특히 관련 업계는 디지털치료제 유효성과 안전성, 비용 효과성 등을 입증할 대조군·유사행위가 명확하지 않은만큼 임상시험에서 출시까지 시판 전·후 RWE 활용 가능성이 높을 것으로 예상하고 있는 상황이다.
실제로 식약처가 지난 2020년 8월 발간한 디지털치료기기 허가심사 가이드라인에서는 허가 이후에도 제품의 잠재적 유익성과 위해성을 모니터링하기 위한 목적으로 실제 임상환경 실사용증거(RWE) 자료를 마련하고 필요 시 제출을 권고할 수 있다.
업계 관계자는 RWE 활용을 위한 데이터 베이스 관련 제도 개선이 필요하다는 입장이다. 그에 따르면 데이터베이스 목록에는 △의료기기 기본정보 △데이터 수집 경로와 분석 알고리즘 △데이터 분석자 정보 등이 포함되며 비용효과성 평가를 위해서는 △현행 수가 △제조원가 등이 포함되기도 한다.
관계자는 "SW기반 디지털 치료기기는 출시 이후 △비용 효과성 △임상효과에 대한 사후관리가 필요하고 수집된 정보들이 웹 기반 데이터베이스에 반영되는 만큼 데이터베이스 변경이 잦을 것"이라며 "RWE 활용을 위해서는 DB 수정에 대한 선제적인 가이드라인 제공이나 신청·승인에서 신고 등으로 간소화 하는 방안이 필요하다"라고 설명했다.
식약처는 제도 개선 필요성을 인지하고 있으며 방안을 검토하고 있다는 입장이다.
여기에 건강보험심사평가원이 9월 발표한 '디지털 치료기기 개념과 건강보험 적용 가능성 검토' 보고서에서 언급된 협의체 구성도 고려대상으로 확인됐다.
당시 보고서에서는 실사용 증거 수집 및 보상체계 설정 등 이해관계자 구체적 논의 대상인 △선정기준 △등재 목록관리 △수집 데이터 기준 △모니터링 기준 △원가 및 수가 설정기준 △평가기간 △가치 측정 방법 협의체 구성이 필요하다고 제안하고 있다.
식약처 관계자는 "디지털 치료기기에서 RWE 활용을 포함한 업계 의견을 지속적으로 청취하고 있다"며 "임상시험 단계에서 RWE 적용 방안이나 관련 협의체 구성 역시 검토 중"이라고 설명했다.
